Avoiding Avoidable Injuries Thread

After popping my ankle last week I decided to start a thread to help others avoid my mistakes.

-when attempting a mounted triangle, be aware that your opponent may roll TOWARDS your bottom foot. This can cause a big problem.
-when hanging out on the mats DON'T post your arm back at an angle behind you. People grappling could roll into that arm and collapse it inward. Yikes!
-when you're on top of your opponent be careful about flattening your hand on the mat when going for an underhook. The guy might trap your hand and roll over. Goodbye fingers. I make a fist now.
-and always be careful when using gi grips. An opponent could roll and twist your fingers.
Can you guys add more to this list?

It is what it is, with or without your reaction.
